I just got a referral

Acknowledge the Referral in HMIS

The referral is not eligible for our housing program

Step 1 – Request a Replacement Referral

Step 2 – Mark the Referral as Unsuccessful in HMIS

I was not able to make any contact with the client

Mark the Referral as Unsuccessful in HMIS, and Remove the Client from the Priority List

I made contact with the client, but we are NOT moving forward

I made contact with the client and intake is completed. We ARE moving forward

Mark the Referral as Successful, Update Current Living Situation, and Enroll the Client in Your Housing Program

The client moved into housing through the CES System

Enter a Housing Move-In Date and Exit the Client from the Priority List

The client exited our program without housing because they are no longer eligible for our program, or they don't want to work with us

Exit the client from your program and update Coordinated Entry Event

The client exited our program without housing because we lost contact with them (jail, treatment, etc)

Exit the client from your program, update Coordinated Entry Event, and remove the client from the priority list
